By completing and submitting this form, you hereby authorize the Law Offices of Bruce Colodny to bill your designated credit card, charges for a consultation at the rate of $150 for 20 minutes (minimum) and thereafter by the minute, at $450 per hour. Charges will include Mr. Colodny’s time required to review records as well as his telephone consultation with you.
